Participation Agreement & Parental Waiver:

I, the undersigned parent or legal guardian, give permission for my child to participate in activities at Slime Time Studios Marblehead ("Slime Time Studios"), including but not limited to slime making, arts and crafts, painting, sensory activities, birthday parties, workshops, camps, and other creative activities.

I understand that participation is voluntary and involves the use of various craft materials, tools, and ingredients.

I acknowledge that participation in these activities carries inherent risks, including but not limited to:

• Slips, trips, and falls

• Cuts, scrapes, bruises, or minor injuries

• Contact with glue, paint, borax solution, activators, clay, pigments, glitter, foam beads, scents, lotions, shaving cream, and other craft materials

• Allergic reactions or skin irritation

• Accidental ingestion of craft materials

• Injuries resulting from the actions of other participants

I voluntarily assume all risks associated with my child's participation.

I understand that Slime Time Studios uses a variety of craft materials and ingredients, including but not limited to:

• School glue

• Borax solution or other slime activators

• Baking soda

• Contact lens solution

• Food coloring and pigments

• Fragrances and essential oils

• Shaving cream

• Lotions

• Glitter

• Foam beads

• Clay

• Paint

• Markers

• Adhesives

• Other common arts and crafts supplies

I acknowledge that my child may come into contact with these materials.

I certify that I have informed Slime Time Studios of any known allergies, sensitivities, medical conditions, or special needs prior to participation.

I understand that Slime Time Studios cannot guarantee an allergen-free environment.

I release and forever discharge Slime Time Studios Marblehead, its owners, employees, instructors, volunteers, contractors, and representatives from any and all claims arising from allergic reactions, skin irritation, respiratory reactions, accidental exposure, or other medical conditions resulting from participation except to the extent caused by gross negligence or willful misconduct.

To the fullest extent permitted by Massachusetts law, I release, waive, discharge, and covenant not to sue Slime Time Studios Marblehead, its owners, officers, employees, instructors, volunteers, contractors, agents, successors, and assigns from any and all liability, claims, demands, causes of action, damages, losses, or expenses arising out of or relating to my child's participation in any activities conducted at Slime Time Studios or on its premises, except where caused by gross negligence or willful misconduct.

This release includes, but is not limited to:

• Personal injury

• Property damage

• Illness

• Allergic reactions

• Accidents

• Emotional distress

• Any other losses occurring during participation.

  
  

In the event of an emergency, I authorize Slime Time Studios staff to obtain emergency medical treatment for my child if I cannot be reached immediately.

I understand that I am solely responsible for all medical expenses incurred.

I certify that my child is physically able to participate in the activities offered.

I agree to supervise my child whenever required by Slime Time Studios policies.

I understand that disruptive or unsafe behavior may result in removal from an activity without refund.

I agree to indemnify and hold harmless Slime Time Studios Marblehead, its owners, employees, instructors, volunteers, contractors, and representatives from any claims, damages, losses, costs, or attorney's fees arising from my child's actions while participating.

This agreement shall be governed by the laws of the Commonwealth of Massachusetts.

If any portion of this agreement is found unenforceable, the remaining provisions shall remain in full force and effect.

I certify that:

• I am the parent or legal guardian of the participant.

• I have carefully read this entire agreement.

• I understand its contents.

• I understand that by signing this document I am giving up certain legal rights on behalf of myself and my child.

• I voluntarily agree to all terms contained herein.

Photo & Video Release:

I grant permission to Slime Time Studios Marblehead to photograph, videotape, or otherwise record my child during participation in activities.

I authorize Slime Time Studios to use these photographs, videos, or recordings in any lawful manner for:

• Advertising

• Marketing

• Social media

• Website content

• Printed materials

• Promotional brochures

• Digital advertising

• Newsletters

• Educational materials

• Future promotional campaigns

I understand that these images may be used without further notice or compensation.

I waive any right to inspect or approve the finished photographs or their use.

I release Slime Time Studios Marblehead from any claims arising from the use of these images.
